CAG: Blebitis is an infection limited to the filtering bleb, whereas bleb-related endophthalmitis is an intraocular infection that includes the vitreous. Inflammatory cells may spill over into the anterior chamber in blebitis, but the vitreous is quiet.
How do you treat Blebitis?
Blebitis can usually be treated with intensive topical antibiotics (such as fortified gentamicin and cefazolin) alone 2. Some surgeons also use parenteral and subconjunctival antibiotics.
Can Blebitis cause blindness?
Blebitis is when the infection is confined to the bleb, and ‘bleb related endophthalmitis’ is when this infection spreads into the eye – it is far more severe and can cause blindness if not treated promptly.
How do you know if a bleb is infected?
A mucopurulent infiltrate, precipitates similar to keratic precipitates, and/or a hypopyon may be visible within the bleb, which is often avascular with thin walls. These signs can be accompanied by a varying degree of anterior chamber reaction and/or a hypopyon, depending on the duration of the blebitis.

What causes eye blebs?
There are various factors which can cause this, which include having a bleb leak, suffering from conjunctivitis or blepharitis, wearing contact lenses, having a previous bleb related infection, having had a second operation on the bleb, using antibiotics, and having diabetes.
What is bleb related endophthalmitis?
If inflammatory or infectious material in a blebitis extend beyond the anterior chamber, the diagnosis is bleb associated endophthalmitis. BAE can have a clinical presentation similar to blebitis, except that the vitreous is involved. A positive vitreal culture is pathognomonic for BAE.
How do you needle a bleb?
Needling of the bleb is performed in the outpatient clinic. After instillation of topical anesthetic and antibiotic drops, a cotton tip applicator soaked in topical anesthetic is applied for 2 minutes to the conjunctival site where the needle will enter. A lid speculum is placed in the eye.
Do eye blebs go away?
Often, blebitis can be cleared without any endophthalmitis developing. If it does occur, patients’ vision rapidly worsens. They get marked redness, pain and conjunctival congestion.
What is bleb failure?
Blebs can fail because the internal ostium is blocked by the iris, a membrane or fibrin and vitreous. Episcleral fibrosis can cause closure of the flap margins lining the bleb and can cause an encapsulation. Collagen within the bleb occurs in 10% to 14% of cases.

No clear management algorithm has been established for bleb associated endophthalmitis. High dose broad spectrum intravenous antibiotics and topical fortified antibiotics form an important part of management. Vitreous tap for identifying organism and its antibiotic sensitivity should be performed and intravitreal antibiotics.
Why can’t we conduct randomized controlled trials for blebitis?
Furthermore, it’s extremely difficult to conduct a randomized controlled trial, because—fortunately—we don’t encounter that many cases of blebitis. When the term blebitis is used, it usually refers to delayed-onset in-fection of the bleb.
What are the risk factors for BAE and blebitis?
The risk factors for BAE and blebitis are similar and numerous, including bleb leakage and the use of anti-metabolites. Inferior blebs are associated with a 7.8% risk per patient-year vs.1.3% risk per patient-year with superior blebs.
